Code the house number and street address of usual residence stated by patient at diagnosis. Do not use PO Box for this data item unless no street address information is availableReview EMR extensively (including driver license) for street address. Record the currently known number and street address of the patient's usual residence.  Leave a blank between numbers and words if space permits. Punctuation should be limited to slashes for fractional addresses (i.e., 103 1/2 MAIN ST) and hyphens (289-01 MONTGOMERY AVE).  Use of the pound sign (#) to designate address units should be avoided whenever possible.  The preferred notation is as follows:  102 MAIN ST APT 408.  Do not use periods after abbreviations. This item is used for the purpose of geocoding and allows referral pattern reports and analysis of cancer clusters or environmental studies. These data may be corrected (if erroneous), but never update the address at diagnosis if the patient moves. Changing this field would destroy its usefulness.  If it is necessary to edit this field, follow the street address guidelines in Item 10060.  

This field is automatically filled in with the address entered in Item 10060 (Current Address) when the case is initially entered in CPDMS.net  Note that if the patient has multiple tumors, the address may be different for subsequent primaries.

This address is a part of the patient's case data and has multiple uses.

  • More than one Residence: Code residence where patient spends majority of time.  If usual residence is not known, code residence the patient specifies at time of diagnosis.
    • Example: Patient is a snow bird and lives in Kentucky 8 months of the year and lives is Florida 4 months of the year.  Use the Kentucky street address as address at diagnosis.
  • Temporary Residents: Code the usual residence rather than temporary address for:
    • Migrant workers
    • Educators on temporary assignment
    • Military personnel on temporary duty
    • Boarding school students (below college level)
    • Persons temporary residing with family while receiving cancer treatment
  • Code residency where student is living (example: dorm) for college students and exchange students.
  • Code address of institution for persons who are:
    • Incarcerated
    • Residing in a nursing, convalescent, and rest homes
    • Long term residents of other hospitals such as Veterans hospital
  • Armed Forces and on Maritime Ships (including Merchant Marine)
    • Code address of military instillation or surrounding community
